Description:
The HealthySteps Core Components eLearning series includes an overview module and seven modules, focusing on each of the HealthySteps Model Core Components.
The modules are designed to inform and support implementation of the HealthySteps Core Components at your practice. Each module includes an overview of the core component, key ingredients, sample workflows, and fidelity and data collection requirements. They also include guidance on the infrastructure, policies, personnel, and resources needed to effectively embed HealthySteps into your practice.
The modules may be
taken all together or separately, depending on the needs of the practice or
health system. They may be taken more than once as a refresher to the model and
to support implementation. The modules vary in length and range from approximately 15-30 minutes each.
Learning Objectives: As a result of taking this course, HS team members will be able to:
- Describe each of the 8 Core Components and explain their relationship to the 3 HealthySteps model tiers;
- Identify key ingredients and questions to consider for each component when developing/revising their site’s implementation plan;
- Identify the data elements and fidelity metrics that correspond to each component; and
- Refer to resource documents from each module that support implementation planning and service delivery for each component.
